CUSP is providing a new mechanism for CUSP faculty to report students who are having academic challenges in their classes. In lieu of sending information through email as you have in the past, we are providing a form via this Website for you to use. Whether you are working with the student individually, you are referring the student to an academic or student services office, or you would like a CUSP staff member to assist you in helping the students, please report any student that is struggling due to academic and/or non-academic issues. There is a place on the form to check how you would like to proceed with the student. No matter how you would to handle the situation, you should have a conversation with the student before completing the form unless the issue pertains to lack of attendance.
There are several areas through the UW system where you can report students who are having academic or personal issues. It can be a challenge to know where to report issues. To assist you in reporting to the correct entity, we have provided links to each area. Contacting the CARE Team vs. Submitting a CUSP Academic Alert
Submit a CUSP Academic Alert when there are concerns about academic performance of CUSP students. Depending on nature of concern, it may also be appropriate to make a CARE Team report at this time. One function of the CARE Team is to be aware of students who may be in distress. Reports from multiple sources on the same student would indicate a higher level of distress and perhaps a different level of intervention. Students who are not in distress, but would benefit from seeing a counselor may check Counseling Services on the CUSP Academic Alert and refer the student to Counseling Services.
The CARE Team Addresses Concerns about “Well-being” Regarding All Students on Campus
Concerns about “well-being” include but are not limited to, concerns about behavior, mood, or decline in academic performance. If you are teaching in other academic programs, you should report your issues to the program and/or the CARE Team. Additional examples may be found in the guide, “Identifying and Responding to Students in Distress”. The CARE Team often serves as an approachable gateway to mental health counseling, student conduct, and other relevant services as well as early intervention with at-risk students. Student Misconduct Including Issues of Plagiarism and Academic Dishonesty
When faculty suspect academic misconduct, three steps are required to Report Student Misconduct. Once a Student Conduct Incident Report is filed, the Office of Student Services coordinates the conduct process including Student Conduct Hearings held on the UW Bothell campus.
