BNURS 409 Requirements and Logistics

The Course

BNURS 409  Partnerships in Community Health (5)
Examine theories, concepts, and practices related to community health care, health promotion, disease prevention, public health, and social justice.  Explore socio-cultural, epidemiological, economic, and political issues influencing local, national, and global health and health care delivery.

Requirements submitted at your Orientation Session

We must have detailed records on file for you in order for you to register for BNURS 409.  We collect these records at your Orientation session prior to the start of your first quarter in the program.  We will need to collect updates of certain documents later on if anything expires before the end of the quarter that you will be enrolled in BNURS 409.  This list is revised every year.  Submission of copies of the following items for BNURS 409 at your Orientation session is required:

  • Your RN license
  • School of Nursing Conviction/Criminal History form
  • Completion of a background check from Verified Credentials. The instructions for completing this check are available here.
  • Your CPR certificate. This is good for two years.
  • Your Td vaccination. This is good for 10 years.
  • Your Varicella/Chickenpox immunization (2 doses 1 month apart), or positive titer. A written note from you indicating that you have had the disease is also acceptable.
  • Measles (Rubeola) record of 2 dose vaccine, given after 1968 without immune globulin (1 dose if born prior to 1957), or positive titer
  • Rubella immunization or positive titer
  • Mumps immunization or positive titer
  • Hepatitis B positive titer results, OR 3-dose series PLUS post-vaccination positive titer, OR submission of waiver form.
  • TB test within one year. If PPD is positive for any reason, or you know that you will always test positive, bring records specifying the test results, your chest x-ray report, and details of prescribed medication, if any.

HIPAA

You will also need to complete UW HIPAA training during your first quarter in the program.  If you have completed UW HIPAA training already, please let your advisor know.  UW HIPAA training should not be done until your advisor has registered you for the training; your academic advisor will contact you once this occurs.  UW HIPAA training instructions are available here

Registration Logistics

Close to the quarter that 409 will be offered, the 409 course coordinator will come in and speak to your class regarding the specific fieldwork opportunities you will have at different sites.  Once you have submitted all of your required documentation and completed HIPAA training, your advisor will ask you to submit a list of your top site choices.  Once you have been assigned to a location, you will receive an entry code allowing you to register for the course.
