Internship Forms and Application Procedures

 Form Procedure 
Letter of Recommendation Ask Instructor to complete online form or submit a formal letter to Internship Advisor in UW1 169.
Resume Submit resume to Internship Advisor in UW1 169.
Application Submit online.
Internship Confirmation Notice Submit online.
Learning Contract (pdf) Must be signed by employer, student, instructor and Internship Advisor. Student responsible for completing contract and getting all signatures.
Student Mid-term Evaluation Submit online form. Must be completed by student and submitted to instructor.
Student Final Evaluation Submit online along with final portfolio. Must be complete to receive credit for course.
