Evaluate a community-based learning student>> This form is for community-based organizations and utilizes a Lickert scale to evaluate each community-based learning student's work with their organization—it is a vital component of a student's assessment and an important tool for maintaining good relationships with our community partners.
Evaluate a community-based learning program>> This form is divided into three sections evaluating: 1) students, 2) the UW Bothell and 3) community-based learning in general. It offers ample room for individualized comments and provides information about the ways in which the programs at UW Bothell intersect with the community it serves.
