Download and print the Recommendation Form. Fill out Section A, then give the forms to the people writing the recommendations, along with a stamped envelope addressed to:

Three recommendations are required for the Nursing and T-Cert programs.
As long as you enroll at UW Bothell as a Pre-Nursing student and are in active status, you do not need to submit an additional online application. You will be required to complete and submit an internal change of major form to the Registrar’s Office. This form is available through your Nursing program advisor.
